The following forms are for Extended, Professional and Continuing Education (EPaCE) students only. All forms for Cal Poly matriculated students can be found at the Office of the Registrar.

| EPaCE Credit/No Credit Grading Option | Use this form to request that an EPaCE course be graded on a "credit/no credit" basis. |
| EPaCE Course Repeat Petition Form | Use this form to petition to exceed the course repeat limitation for an EPaCE course. |
| EPaCE FERPA Release | Use this form to release your educational records to a third party. |
| EPaCE Refund Request | Email extended@calpoly.edu to request a refund if you have a credit on your Extend Ed account. |
| EPaCE Petition to Withdraw | Email extended@calpoly.edu to petition to withdraw from an EPaCE course. |
